If you would like to volunteer in our pre-school, day-school, children's ministry, youth ministry, or adult ministry, you need to fill out the following risk-management form. By filling out the form in its entirity you are giving Immanuel Lutheran permission to run a back-ground check upon you. This check is good for 5 years at the most, but may be required to be redone earlier, at which time, you will need to resubmit this form.

This allows an Immanuel Representative to perform an I-Chat background check upon you. This confidential check will be shared with the ministry supervisor alerting them if you are allowed or not allowed to volunteer in a desired ministry area.
